Preparing for Adulthood Questionnaire (For Carers of young people)

We are developing a new Preparing for Adulthood (PFA) protocol and pathway document. The existing Preparing for Adulthood Protocol and Pathway document was produced in 2016 and is due for an update. We are adopting a coproduction approach because we value people with SEND and their families as equal partners who are experts by experience. We want to co-design our PFA protocol and pathway to ensure that it is meaningful and helpful.
We would be grateful if you could complete this questionnaire to let us know your opinion on how best to support young people and their families.
