Young Carers Questionnaire

Do you help to take care of someone in your family who has a long term health condition, disability, mental health problem or drug and alcohol problem?

Are you under 18?

If so, you are a Young Carer and we need your opinion.

Nottinghamshire Carers Association are carrying out a short questionnaire on behalf of Nottinghamshire County Council.

We want you to help us to understand what kinds of services Young Carers would like and how best to support you. Your answers will be used to help make services for Young Carers like you.
