Living with a long term condition


The Living Well with a Long Term Condition (LWLTC)Programme has new dates for 2019.

The Living with A Long Term Condition (LWLTC) programme is for anyone living in Derbyshire who has one or more long-term health conditions such as asthma, arthritis, Multiple Sclerosis (MS), depression, diabetes, Chronic Obstructive Pulmonary Disease (COPD), heart disease, sickle cell, HIV, ME/chronic fatigue, chronic pain or any other condition. The course is also available and proven beneficial for carers of someone living with a long term condition. The course is designed to help participants cope with their health condition and improve their quality of life.

LWLTC is a free 6-week course that can help a person to take more control of their health by learning new skills and strategies to manage their condition on a daily basis. The course focuses on what a person can do, rather than what they can’t. Each session is one day a week and lasts two and a half hours. There is one session a week and refreshments are provided.

Topics covered include:

Dealing with symptoms like pain and tiredness
Coping with depression or feeling low
Relaxation techniques
Physical activity
Healthy eating
Communicating with family, friends and health professionals
Planning for the future
Setting realistic goals and pacing yourself

The course is run by two trained tutors who themselves live with long-term conditions. They can give practical advice based on their own life experiences.
