Patient Questionnaire - Pharmacy First Consultation
There are two short questionnaires to fill out the first one is around Emergency Supply Service. This service is provided by multiple pharmacies across Nottinghamshire and continues to help patients to access medicines which they have run out of in a timely manner when they are unable to obtain a supply of their prescription in the out of hours period or other emergency situations.

The second questionnaire is the Pharmacy First service which is a service provided by multiple pharmacies across Nottinghamshire and continues to help patients to access appropriate advice and treatment for minor ailments in a timely manner and reduce demand for GP appointments.

The six week consultation period opens on Monday 28th February 2022 and closes on Sunday 10th April 2022
