Nottinghamshire Dementia Review


Nottingham and Nottinghamshire CCG / ICS are working with patients, carers, health and care professionals and wider stakeholders to review the Dementia pathway. The aim of this review is to help shape the future service provision for the Nottingham and Nottinghamshire Dementia Well Pathway. 

To support this review a period of engagement is taking place between 17 May 2021 (Dementia Awareness Week) and the 16 July 2021 to understand the experiences of all those affected by Dementia.

 

They are asking you to participate in an online survey: https://www.surveymonkey.co.uk/r/66YWD5S

 

This survey is open to patients, carers, members of the public, health and social care colleagues, charities, communities and networks.  The survey is quick and easy to complete and will take no more than 10 minutes of your time. If for whatever reason, you cannot complete this survey or would like to take part in a phone interview instead, kindly contact Katie Swinburn at katie.swinburn@nhs.net to arrange a mutually convenient time and date to take part. 

They will also be arranging for some virtual focus groups to take place in the very near future.  If this is something that you would be interested in participating in then please contact Katie Swinburn on the above email address who will register your interest and keep you up to date. 

 

They are also keen to talk to any groups who are interested and would like to hear more about the work that we are doing.  If you are currently meeting either virtually or in a community setting then please do contact Katie Swinburn in order that they can arrange to attend.
